NURS545 Population Health Management for Nurse Leaders

This course, designed for the MSN advanced nursing practice specialty student, explores the principles behind the healthcare delivery method shift from volume-based to value-based population health management approach to care which pivots away from sick care and towards well care with full consideration of social determinants of health. This course prompts the emerging nurse manager to reimage healthcare delivery practices by identifying population care gaps, interpreting population panels, and engaging with interdisciplinary healthcare team members to identify and implement evidence-based interventions in an effort to close population care gaps.
